Acceptance and Scope

These Website Terms of Use govern access to and use of VitalReplenish.com and its public-facing online services. By using the website, visitors agree to these terms. These terms apply only to website use and do not constitute consent to medical treatment, authorize payment-card charges or enroll anyone in marketing communications.

No Provider-Patient Relationship

Using the website, submitting an appointment request or receiving general information does not establish a provider-patient relationship or constitute consent to treatment. A provider-patient relationship is established only after required registration, clinical screening, acceptance by an authorized healthcare provider and completion of the appropriate consent documents.

Submitting an appointment request does not guarantee availability, approval or treatment. IV hydration and injectable services require clinical screening and provider approval.

Educational Information and Medical Disclaimer

Website content is provided for educational and informational purposes only. It is not individualized medical advice and should not replace evaluation, diagnosis, advice or treatment from a qualified healthcare professional.

Descriptions of ingredients, therapies, wellness goals or potential benefits are general. Individual suitability and results vary. A licensed healthcare provider must determine whether a requested service is appropriate after reviewing the patient’s health information.

Vital Replenish does not provide emergency services. Anyone experiencing a medical emergency should call 911 or visit the nearest emergency department.

Appointments and Clinical Approval

Appointment requests are subject to confirmation. Vital Replenish may decline, postpone, modify or cancel a requested service when clinically appropriate, when required information has not been provided, when a service is unavailable or when providing it would conflict with applicable law, professional standards or clinic policy.

Deposits, Cancellations and Refunds

A $50 deposit is required to reserve an appointment. Appointments must be cancelled or rescheduled at least 24 hours in advance. For a late cancellation or no-show, the deposit may be retained as a cancellation fee.

Clinical screening and provider authorization must occur before an IV solution or injection is prepared. Once preparation has begun specifically for an approved patient, the associated payment may be nonrefundable to the extent permitted by law. Services already rendered are nonrefundable except where required by law or approved by Vital Replenish.

Payments and Billing Questions

Payments may be processed through a third-party payment provider. These Website Terms do not authorize Vital Replenish to charge a stored payment card for future balances. Any stored-card, balance-billing or recurring-payment authorization must be obtained separately.

Communications

When someone contacts Vital Replenish or requests an appointment, Vital Replenish may respond using the contact information provided. Automated, recurring and marketing messages require any separate consent presented during the opt-in process.

Consent to receive marketing text messages is not a condition of purchasing goods or services. Message frequency varies. Message and data rates may apply. Where supported, recipients may reply STOP to opt out and HELP for assistance.

Privacy and Health Information

Information collected through the website is governed by the Website Privacy Policy. Uses and disclosures of protected health information are governed by the applicable HIPAA Notice of Privacy Practices.

Visitors should not submit urgent medical concerns through ordinary email or a general website contact field.
